To place an item on reserve for a course…

  1. Submit the form below.

Instructors in Redmond, Madras, or Prineville

  1. Download, complete, and print thepaper Course Reserves Form.
  2. Bring your completed form and item to the Front Desk of your campus.

Course Information

Enter the course catalog number (e.g., WR 121).
Select the term(s) this course will be taught.
Items will be removed from Reserves after this date. The selected date must be less than two years in the future.

Reserve Item #1

Items borrowed from other libraries through ILL or Summit cannot be placed on reserve.
Select the period of time students may check out this item.

Terms

  • Please allow up to 1 week for reserve materials to be processed.
  • Reserve items will be processed in the order received. Additional time is required at the beginning of terms due to high volumes of submissions.
  • The Library affixes a removable label, barcode, and security strip to copies of materials submitted.
  • The Library is not responsible for replacing damaged, stolen, or lost materials placed on reserve.
  • All items will be pulled at the end of the period specified on this form. Personally and departmentally owned materials will be returned via campus mail.
